THE VALUE OF PHIGFBP-1 IN RULING OUT PRETERM BIRTH AMONG WOMEN WITH THREATENED PRETERM LABOR AT CAN THO OBSTETRICS AND GYNECOLOGY HOSPITAL

Abstract
Background: Threatened preterm labor is a common problem in obstetric practice; however, predicting progression to actual preterm birth remains challenging. Phosphorylated insulin-like growth factor-binding protein-1 (phIGFBP-1) is a biomarker used to predict preterm birth, as it appears in cervicovaginal secretions after early separation between the chorion and decidua. Objectives: 1. To describe the clinical and paraclinical characteristics of pregnant women with threatened preterm labor; 2. To determine the value of phIGFBP-1 in ruling out preterm birth. Materials and methods: A prospective cross-sectional study was conducted on 203 pregnant women diagnosed with threatened preterm labor between 28 and less than 36 weeks of gestation. All participants underwent phIGFBP-1 testing and were followed until delivery to assess pregnancy outcomes. Results: The mean gestational age at admission was 33.27 ± 2.19 weeks. Abdominal pain was the most common reason for admission (85.2%). A closed cervix was observed in 65.5% of cases, while 63.1% had uterine contractions and 61.6% had a cervical length >25 mm. The rate of positive phIGFBP-1 tests was 37.4%. Women with a negative phIGFBP-1 test were more likely to remain undelivered within 7 days (OR=21.96, 95%CI:8.07-59.77; p<0.001) and within 14 days (OR=17.21, 95%CI: 8.31-35.63; p<0.001) compared with those with a positive test. Conclusion: The phIGFBP-1 test is a valuable tool for ruling out imminent preterm birth, particularly within 7– 14 days after testing in women presenting with threatened preterm labor.
