STUDY ON CLINICAL CHARACTERISTICS AND TREATMENT OUTCOMES OF HAND ECZEMA WITH TOPICAL BETAMETHASONE DIPROPIONATE AT DONG NAI PROVINCIAL DERMATOLOGY HOSPITAL IN 2025-2026

Abstract
Background: Hand eczema is a common dermatological condition closely associated with occupational factors , with a tendency toward chronic progression in most patients. It presents with varying degrees of pruritus, pain, and burning sensation, significantly impairing quality of life. Diagnosis and management remain challenging, necessitating the avoidance of triggers alongside topical or systemic anti-inflammatory treatments when indicated. Among these, betamethasone dipropionate is a potent topical corticosteroid that rapidly alleviates inflammation and itching; however, it must be used judiciously to minimize adverse effects. Objectives: To describe clinical characteristics and to evaluate the outcomes of topical betamethasone dipropionate treatment for hand eczema at Dong Nai Provincial Dermatology Hospital in 2025-2026. Materials and methods: Case series was carried out on 44 hand eczema patients treated at Dong Nai Provincial Dermatology Hospital in 2025-2026. Results: 50% of patients had a history of atopic dermatitis. The most common clinical manifestations were scaling (84.1%) and erythema (70.5%). The predominant clinical subtypes were irritant/allergic contact dermatitis (75%) and hyperkeratotic eczema (61.4%). After 6 weeks of treatment, the mean Hand Eczema Severity Index (HECSI) score significantly decreased from 15.07 to 11.07 (p < 0.001). Conclusions: The predominant primary lesions of hand eczema were scaling and erythema. The most common clinical subtypes were irritant/allergic contact dermatitis and hyperkeratotic eczema. Most patients had moderate disease severity. Treatment outcomes demonstrated that topical Betamethasone dipropionate was effective in managing mild to moderate hand eczema.
